Blood Engine is an action game. It is a third-person, open-world experience set in a dystopian future, specifically the year 2058. The game revolves around a world facing overpopulation and dwindling resources, where a catastrophic experiment at a covert mining and research facility leads to the creation of the titular Blood Engine – a soldier enhanced with a metallic bacteria designed to be the ultimate warrior.

In Blood Engine, you’ll take on the role of a Blood Engine soldier who awakens in the midst of this chaos inside the now-infested facility. The game's narrative focuses on your fight against the Red Swarm, efforts to rescue survivors, and the attempt to prevent the swarm from causing a global catastrophe.

The gameplay is characterized by intense combat, where you face off against disfigured biomechanical enemies using a variety of weapons and explosives. However, the game is noted for its mature content, including graphic violence, blood, and gore, although it avoids depictions of sexual content, drugs, alcohol, smoking, or swearing.
