MyLATITUDE™ is a remote monitoring system that gives your health care provider access to your implanted device data. It allows patients to manage their medical information from a remote location. This app is intended for use by patients with a LATITUDE™ NXT Communicator, a remote monitoring device that connects to an audio intercom.

Use this app to:

• Set up and manage your LATITUDE™ NXT Communicator
• Receive alerts about the status of your device and know what to do next
• Access your medical information from a remote location

This app is not intended for use with any other audio or video equipment. The app does not provide continuous monitoring of the patient. Use this app only for informational purposes. It is not intended for use in the diagnosis of any disease or other condition or the cure, mitigation, treatment, or prevention of any disease or other condition.

It is intended for use only with the LATITUDE™ NXT Communicator and with the latest version of this app.
