This app provides a highly secure platform for patients to record their INR results, vital signs, and other relevant home test results anytime, anywhere. The app is designed to let you take greater control of your care by communicating with your healthcare team during your clinic’s hours of operation. You can also view dosing information, track your progress via a graphic chart, view your complete self-test history, and order supplies.

With the ACH HealthCheck app, your test results are sent directly to your physician or clinic in real-time. You can choose to receive updates using push notifications or email (if provided by your clinic). The app is developed under physician-selected settings tailored to you, and your personal identifiable information will not be sold or otherwise marketed to third parties.
