This is an application for the surveillance of notifiable medical conditions (NMC). NMC is a new concept in public health. It is a condition that must be reported in order to receive a medical certificate and, if required, to receive public assistance. It is defined as a health condition that is likely to lead to death, or which puts a person at risk of developing a life-threatening illness, or which leads to serious or significant disability or loss of function. NMC is one of the conditions that will be included in the second phase of the UK's National Health Service (NHS) Health Screening Programme, which is currently being developed.
