My Health Planner, developed by My Health Toolkit LLC, is a free care management tool available for iPhone and iPad users who are members of BlueCross BlueShield of South Carolina, BlueChoice Health Plan, or the Blue Cross and Blue Shield Service Benefit Plan. With My Health Planner, members can easily connect with their care team through private messaging and set reminders to help remember their medications. The app also tracks progress over time, including steps taken, and is fully HIPAA compliant to ensure maximum privacy and security.

The app is available for all medical and dental benefit plans administered by BlueCross BlueShield of South Carolina and BlueChoice Health Plan, as well as some large employer plans administered on behalf of other Blue Cross and Blue Shield licensees. Unfortunately, the app is not available for Medicaid members.

My Health Planner is a convenient and accessible tool for BlueCross BlueShield members to manage their health and connect with their care team in a secure and private way. The app can be downloaded for free from the App Store using the access code provided by the care team and insurance card details.
