Modello 4 is a free Android app developed by Centro Servizi Nazionale IZSAM. The app can only be downloaded by users with an account to access the VETINFO portal and enabled to access the animal registry office. The app is designed to manage the electronic Modello 4 form for farms under the user's responsibility.

The Modello 4 form is a declaration of origin and destination that accompanies the animal during transportation. It contains information about identification, treatments, food chain, destination, transportation data, health certificate of the veterinarian who visited the animals before the move, and identification elements of the moved animals.

The app is an extension of the web application for managing national animal registry offices, and it allows authorized users (the holder or their delegate in charge of the animal registry office, or their sub-delegate in charge of filling out the Modello 4 form) to fill out the form directly in the field for the farm under their responsibility and register it in the system. The app currently supports electronic Modello 4 forms for bovine, buffalo, ovine, caprine, swine (only batch movements), and equine species. The extension to other animal species is expected in future versions of the application.
