Dr. Fadil and his neighbors live in a small village named Al-Nasali, which is situated in the mountains, where Dr. Fadil is the only physician, and the people only have him for their doctor, as there is no other medical facility in the village, and the people cannot afford to go to a hospital, so they depend on him for their health.

The story starts with the end of the novel The Rules of Two Neighbors, which ended with the death of the child named Adam, and his uncle and grandfather trying to find the child to take care of him. But they could not find him, so they decided to travel to Berha, the farthest village in Gartinia, in order to find him, and on their way, they are attacked by a group of men, who take them to a place, and kill them, and take the child. So the story begins with the death of the child, and the search for him by Dr. Fadil.
