HubChart Telemed is a free iPhone app designed to facilitate patient-centered collaborative care. This HIPAA compliant app allows care teams to securely share vital information with each other, anytime and anywhere. With HubChart, care teams can create patient-named collaborative chat groups that function like ad-hoc health records. Within these secure groups, participants can share chats, files, images, videos, and links, which are archived for future reference.

The app is especially valuable for complex medical care, case management, and concierge medicine, where a multidisciplinary team continuously coordinates to provide optimum care. The care team can consist of doctors, doctor's staff, the patient or family, and even the pharmacy. HubChart's patient-centric groups enable individualized care delivery and help expedite care delivery.
